The role of this position(s) is to :
Chore : provide the above day to day household maintenance tasks that are needed to maintain a consumer's home in a clean, sanitary and safe manner. This service includes heavy household chores such as washing floors, windows and walls, securing loose rugs or tiles, moving furniture to provide safe access and egress.
Homemaker : provide housekeeping, shopping, laundry, essential errands, basic meal preparation and meal planning (not for special diets), maintain a safe environment, observing and monitoring and reporting on consumer condition and teaching those tasks.
Companion : provide non-medical care, supervision and socialization to a consumer. Assist and / or supervise a consumer with light household tasks such as meal preparation, laundry and shopping. Provide services designed to develop, maintain and / or maximize the participant's independent living skills, to achieve objective of improved health and welfare to support the ability of the participant to establish and maintain a residence in the community.
Individual Support and Community Habilitation : Provides services which may include locating appropriate housing, the acquisition, retention or improvement of skills related to personal finance, health, shopping, and use of community resources, community safety, and other social and adaptive skills to live in the community. Individual support and community habilitation provide supports necessary for the individual to establish, live in and maintain a household of their choosing in the community. It may include training and education in self-determination and self-advocacy to enable the individual to acquire skills to exercise control and responsibility over the services and supports they receive, and to become more independent, integrated, and productive in their communities.
Personal Care Worker : This position is to provide a range of personal care assistance that is appropriate and necessary for the participant's health and well-being. To enable participant to accomplish fundamental activities of daily living including but not limited to eating, toileting, dressing, bathing, transferring and ambulation.
QUALIFICATIONS : A High School Diploma / GED or life / work experience with providing care to individuals with disabilities or the elderly. Ability to complete all initial and ongoing background checks including but not limited to : CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and / or certification checks (i.e. nursing). Valid US driver's license and ready access to a vehicle.
TRAINING REQUIREMENTS : Must maintain all certification needed in the program including : CPR, First Aid, and BFAIR Human Rights Training. May need to attend workshops, seminars or BFAIR functions as needed for job development.
RESPONSIBILITIES :
- Treat each consumer, co-worker, supervisor and others with the respect and dignity due every human being, and adhere to the Human Rights Standards and the Elder Abuse Reporting Guidelines through the Executive Office of Elder Affairs (EOEA) around physical and emotional abuse, including the timely reporting of violations. In addition, will abide by the Disabled Persons Protection Commission's (DPPC) Mandated Reporter Guidelines.
- Set appropriate limits with consumers and problem solve independently.
- Must exercise good judgment, including the appropriate handling of emergency situations.
- Must communicate effectively, both verbally and in writing, with consumers, families, co- workers and supervisor.
- Act as an appropriate role model through dress, speech and actions.
- Be able to monitor and effectively report, verbally and in writing on a consumer's progress, challenges and status.
- Work individually with any program participant, which may include : (N / A for Chore / Homemaker) Assistance and / or supervision with mobility (light to heavy lifting and transfers), recreational activities or other needs.
- Effectively perform crisis intervention which may include : First Aid / CPR, fire evacuation, disaster relocation or emergency search.
- Arrange all staff PTO time off requests with Director. Vacation time must be requested at least two weeks prior to the desired date(s). In the case of illness you must call out to your supervisor two hours prior to the start of your shift.
- Must arrive on time to scheduled shifts and remain on duty until proper coverage is found.
- Attend scheduled staff meetings and trainings as assigned; work shifts as assigned. Shifts may not be changed unless permission is received from supervisor prior to change.
- Required to maintain certifications in all agency mandated trainings.
- Accurately documents all hours worked on the Agency electronic time management system. All timesheets must be submitted timely for approval which act as your electronic signature and approval. Time sheets should reflect employee's assigned scheduled unless prior approval was given by supervisor to alter assigned schedule.
- Responsible for maintaining current certification(s) including but not limited to START, CPR / FA, and MAP (if applicable to position).
- Responsible for completing all training assigned in electronic Learning Management System on time or attending any scheduled training. If unable to complete or attend it is the employee's responsibility to obtain prior authorization / permission to re-schedule training from Supervisor and notify Human Resources.
- Responsible for routinely checking electronic communications; including but not limited to time off management, BFAIR email, BFAIR portal, and electronic LMS system. These systems should be checked each shift your work.
- Meet with Director or designee monthly for individual supervision; attend staff meetings, and attend other meetings as assigned.
- Adhere to agency / personnel policies, regulatory agency(s) regulations, follow emergency procedures and maintain individual's confidentiality in accordance with HIPPA standards.
- The agency reserves the right to revise existing schedules and / or program sites assignments, with notice due to the changing needs of the individuals served and program(s).
- Perform other duties as assigned by Director and / or designee.
