Job Responsibilities
- Maintain positive partnerships with referring agencies, ensuring timely and effective communication regarding case activities.
- Provide crisis intervention services for youth experiencing acute distress and dysfunctional behaviors.
- Deliver supportive case work counseling to youth and families, including assessments, treatment plans, and discharge planning.
- Coordinate psycho-educational groups as needed.
- Ensure youths' home visits are goal-oriented and focused on building trust, communication, and healthy relationships.
