Job Summary
The Community Health Nurse (CHN) provides, teaches, supervises and documents nursing care of the sick and injured at home. The CHN intervenes to promote, maintain or restore health to prevent illness and to effect rehabilitation. The CHN collaborates with other healthcare providers, professionals and community representatives to coordinate care at home. The CHN evaluates the individuals and family's response to interventions. In order to effectively provide home care, they must have a working knowledge of home care reimbursement and regulatory guidelines.
Experience
- One-year acute nursing care preferred; hospital based preferred.
- Experience in home care setting strongly preferred.
- Demonstrated knowledge of the principles and practices of nursing.
- Knowledge of and ability to apply community health principles and practice.
- Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
- Experience with self-scheduling of caseload preferred.
- History of flexibility in assuming varied assignments.
Education and Training
- Registered Nurse required.
- Bachelor of Science in Nursing recommended.
- Graduate of a N.L.N. accredited nursing program.
License, Certification & Registration
- RN License - Current license as a professional nurse within MA.
- Active and valid driver's license.
- CPR required every two years.

Schedule
8:00 am- 4:30pm, variable days
