What are the responsibilities and job description for the Hospice Chaplain - PRN position at Bernardo Hospice Care?
- Complete spiritual assessments on all patient and/or families within five (5) calendar days of referral
- Determine the frequency of routine visits and/or contacts to be made for each patient and/or family based on the initial assessment as well as ongoing assessments of the patient and/or family
- Provide routine visits with patients and/or families with chaplain referrals and completed chaplain assessments
- Provide direct spiritual support/counseling and end of life counseling to patients and/or families in keeping with the personal spiritual beliefs of the patient and/or family
- Provide guidance and assistance as needed/requested with funeral arrangements, plans, and/or conducting services
- Work with the interdisciplinary team, other clergy and community groups to enhance his/her sensitivity to the spiritual concerns of patients and/or families experiencing terminal illness and/or loss
- Provide bereavement counseling as need and directed by the Clinical Director and Bereavement Coordinator
- Complete necessary paperwork/reports accurately and timely
- Attend as a participate in the Interdisciplinary Team (IDT) Meetings, staff meetings (routine and called), and in-services
- Assist with and provide staff in-services related to the Chaplain program/services and/or spirituality issues or related field for the hospice team members and/or other facility staff when appropriate
- Assist with community programs when required (e.g. communion services in facilities)
- Provide confidential counseling services to hospice team members as appropriate
- Communicate regularly with other hospice team members (RNCM, Hospice Aide, Social Workers, Volunteer Coordinator, Bereavement Coordinator) to provide patient updates, patient needs and/or concerns and to assist in developing a multi-disciplined plan for working effectively with the patient and/or family
- Maintain a continued sensitivity and awareness to issues of diversity such as, but not limited to cultural, sexual, age, economic status and lifestyle and maintain a working knowledge of the hospice model of care
- Perform other job-related activities as assigned.
Pay: $25.00 per hour
