Early On Service Coordinator – One Year Temporary Position
Start Date: 2019-2020 School Year
Qualifications:
- Certification as a School Social Worker
- Preferred knowledge of Early On programs
- Demonstrated knowledge of child development
- Demonstrated knowledge of evaluation of family needs and priorities
- Demonstrated competencies on areas of interpersonal relationships, communication, and organization
Performance Responsibilities:
- Evaluate family needs in the child's home or preferred site
- Prepare for and participate in IEP/IFSP process
- Collaborate with parents, teachers, administrators, and other ancillary and agency staff
- Maintain accurate and timely records
- Serve as service coordinator to eligible infant/toddlers and their families
- Demonstrate professional conduct and exhibit professional growth
- Effectively plan and organize duties
- Comply with school district policies and procedures
- Effectively participate in meetings, committees, staffings and other required district and building activities
- Coordinate services with other community human service agencies
- Comply with Medicaid billing procedures
Reports to: Supervisor of Birth to Three
Terms of Employment: In accord with the Master Agreement
