Pay is commensurate with experience within a range of $20.00 - $26.00 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibility of the Child & Family Services Case Manager will be case management services, resource acquisition, crisis intervention, and outreach activities to specified youth and their families.
Specific duties including but not limited to:
- providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies;
- delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis;
- providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP;
- responding to crises while on call in rotation with other similarly trained staff.
- regular and predictable attendance is an essential function of the job.
- Case manager is expected to meet regularly with each QMHP directing the treatment plans for their clients at least once monthly to review cases and ensure direction of clinical services.
- Case manager is expected to contact a QMHP if urgent clinical consultation is required. Case managers will be notified of who the available QMHP is for each week.
- Additional responsibilities may be assigned as circumstances warrant.
Qualifications: The Child & Family Services Case Manager position requires:
Including but not limited to:
- a BA/BS degree or four years of equivalent education and/or experience working in the human services field.
- reliable transportation and automobile insurance; and a valid, unrestricted driver's license;
- demonstrated strong interpersonal skills in working with diverse publics, consumers, and referral agents;
- flexibility in assigned working hours;
- a commitment to interdisciplinary and community mental health center approaches; and
- an energetic and positive approach to the rapidly evolving changes and challenges of a complex workplace.
