Overview
We are seeking a dedicated and knowledgeable Care Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate medical services and support throughout their treatment journey. This position requires a strong understanding of medical terminology, acute care processes, and effective utilization management strategies. The role would also include providing telehealth support for our providers at long-term care facilities.
Responsibilities
- Conduct comprehensive diagnostic evaluations to assess patient needs and develop individualized care plans.
- Collaborate with multidisciplinary teams to coordinate inpatient care and ensure seamless transitions between different levels of care.
- Monitor patient progress and adjust care plans as necessary, utilizing knowledge of physiology to inform decision-making.
- Provide education and support to patients and their families regarding treatment options and health management strategies.
- Implement utilization management practices to optimize resource use while maintaining high-quality patient care.
- Maintain accurate documentation of patient interactions, care plans, and progress notes in compliance with medical standards.
- Stay informed about current best practices in acute care and infectious disease management to enhance patient outcomes.
Qualifications
- Medical Assistant or Certified nursing assistant required; experience in acute care settings preferred.
- Strong knowledge of medical terminology and physiology.
- Experience with diagnostic evaluations and inpatient care management is highly desirable.
- Familiarity with utilization management principles and practices.
- Ability to work collaboratively within a multidisciplinary team environment.
- Excellent communication skills, both verbal and written, for effective patient education and team coordination.
