Job Summary
An intake coordinator plays a crucial role in ensuring that patients receive the highest quality of care from the moment they sign up for our hospice services. This position involves managing patient intake processes, maintaining accurate health information, and ensuring compliance with HIPAA regulations. The ideal candidate will be a licensed LVN with previous hospice experience, a background in health information management, and excellent phone etiquette to facilitate effective communication with patients and healthcare providers.
Responsibilities
- Conduct initial patient assessments and gather necessary health information for accurate record-keeping.
- Manage appointment scheduling and coordinate patient flow within the clinic.
- Ensure all patient records are compliant with HIPAA regulations and maintain confidentiality at all times.
- Utilize HospiceMD for entering and retrieving patient data.
- Provide exceptional customer service by answering phone calls, addressing inquiries, and resolving issues promptly.
Experience
- MANDATORY - Previous experience in a hospice setting, either on the field with patients or doing QA.
- Familiarity with health information management practices is highly desirable.
- Proficiency in using electronic health record systems like HospiceMD.
- Excellent phone etiquette and communication skills to interact effectively with patients and team members.
