Best Point is seeking an Ohio RISE Care Coordinator to provide family-centered and community-based wraparound services. Ohio RISE Care coordinators work with each family to develop a care plan, including transition plan and crisis plan. Develops and coordinates the wraparound team for each child served based on individual needs and strengths. Participates in after hours, on call response. Conducts regular assessment using the Child and Adolescent Needs and Strengths (CANS). Care coordinators demonstrate commitment to working with diverse populations.

Minimum Education, Experience and Other Skill Requirements:
This position requires a Bachelor’s or Master’s degree in Social Work, Counseling or related field from an accredited college or university plus a minimum of three years experience in working with children and families.
Have a background in family systems, community systems, case management, child and family counseling, child protection or child development. Have qualifications and experience to work with children and families who are experiences serious emotional disturbance, trauma history, co-occurring mental health and substance use disorders and/or those involved with multiple systems of care.
Must be culturally responsive with training and experience to manage complex cases.
Must possess minimum technical skills including familiarity with Microsoft Windows and ability to use standard Microsoft Office applications
