Job Description:
*Care Manager LCSW for our Care Management Team at our Little Company of Mary Torrance Hospital in Torrance, CA. This is a full-time position that will work 8-hour Day shifts.
Required Qualifications:
*Master's Degree - Social Work.
*Coursework/Training - All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and keep their designation in current status.
*California Clinical Social Worker License upon hire.
*2 years experience - Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.
Computer experience.
*Designated as Behavioral Health Clinician to perform 5150 evaluations 18 months of hire.
*CPI certification (nonviolent crisis intervention training) within 1 year of hire.
Preferred Qualifications:
*Discharge planning experience.
