Various locations around Buffalo, Amherst and Orchard Park.
A licensed health care practitioner who provides nursing care under the direction of an RN, NP, physician, physician assistant. LPNs provide compassionate care that is respectful of each patient's needs, values and wishes. The LPN is responsible for monitoring the health status of clients at the Certified Community Behavioral Health Clinic. She/he responds to any emergencies in accordance with established policies and procedures and is responsible and accountable for providing direct and indirect patient care procedures reflective of BestSelf Behavioral Health mission and values.
RESPONSIBILITIES:
Assists with collection of health information
and health monitoring under the supervision of the RN, NP, physician, or
physician assistant.
Coordinates and schedules necessary medical
appointments for clients
Completes urine screen and orders toxicology
and lab reports under the supervision of the RN, NP, physician, or
physician assistant.
Assists in obtaining necessary medical
documentation and coordinating care with other medical providers
Requests prior
authorizations for medications
Provide nursing treatments
and administer medications under the supervision of the RN
Facilitates
groups
Schedule and assigned
duties are subject to change based on the needs of the program and the
clients we serve.
REQUIRED
KNOWLEDGE AND SKILLS:
Must
have basic knowledge of mental health and substance use disorders and the
recovery process, be able to relate well individuals from diverse cultures,
and have good judgment.
Excellent
organizational, problem-solving, and time management skills.
