The dual position will assume the responsibility for outpatient infusion clinic patients as well as provide coverage in central sterile products. The infusion pharmacist collaborates with the infusion services team to deliver high quality coordination of care. The pharmacist is imbedded in the clinic to assist with patient care questions. Infusion pharmacist prescreens and processes infusion orders covering an array of specialties including GI, Rheumatology, Neurology, Pulmonary and Endocrine. The sterile products pharmacist role is to oversee compounding and verification of IV admixtures in compliance with established guidelines and regulations. The position will provide support to a variety of patient populations, across varying levels of acuity and complexity. Sterile products pharmacist will exercise time management, communication and problem solving skills in an exciting and fast paced environment. Pharmacist is supported by a team of pharmacy technicians, clinical and sterile products pharmacists and the on call operations and clinical pager program.

Job Description:

1. Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug  Administration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures. 

2. Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.

3. Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.

4. Provides drug information to physicians, nurses and other health care professionals.  

5. Checks and verifies sterile and non-sterile medications.

6. Supervises the activities of students, technicians, and residents within the pharmacist area of practice. 

7. Promotes a culture of patient safety by submitting  medication related events and near misses.

8. Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.

9. Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.

10. Participates in the onboarding and training of new hires and precepts pharmacy students and/or residents through core clinical rotations or learning experiences.

11. Regularly participates in department initiatives including but not limited to supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources.

12. Supports departmental workgroups to improve process and quality outcomes.

13. Supports the development and revision of departmental clinical guidelines.

14. Other duties as assigned

Required Qualifications:

1. PGY-1 Residency OR 1-3 years relevant hospital experience.

Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.
