About the Role
The Team Nurse position is a critical part of our healthcare team, responsible for providing high-quality direct patient care in an outpatient primary care or family medicine practice. Working collaboratively with providers and medical assistants, the Team Nurse plays a key role in enhancing the safety, effectiveness, and efficiency of patient care.
Key Responsibilities
- Coordinate and facilitate quality clinical management of a broad-based outpatient population across the continuum of care.
- Collaborate with providers to identify needs for preventive care, evidence-based chronic disease management, and care coordination.
- Provide general and specific healthcare education to patients, working directly with them to identify and monitor self-management goals.
- Administer vaccines and medications by parenteral, oral, rectal, or nasal route by orders of the prescribing provider.
The ideal candidate will possess strong interpersonal skills, with excellent patient communication and patient education skills. They will also have advanced skills with Microsoft applications and be able to maintain databases.
