Job Summary
The Case Manager plays a key role in guiding residents through the intake and placement process, as well as the exit and follow-along service process of the transitional housing program for survivors of domestic violence. This position is responsible for overseeing apartment preparation, conducting ongoing residency monitoring, and ensuring residents remain engaged in program requirements. Through comprehensive assessment, individualized planning, and service coordination, the Case Manager supports both residents and their children in addressing their unique needs through trauma-informed practices. The role is dedicated to enhancing residents’ overall well-being, fostering self-sufficiency, and ensuring continuity of care. Additionally, the Case Manager may provide supervision and mentorship to advocates, interns, and volunteers when assigned by the Director of Transitional Housing.
Duties and Responsibilities
Direct Services (Resident Support and Case Management)
- Assist residents during the application, intake, and placement processes.
- Develop and maintain a professional, trauma-informed relationship with each resident family to support their development, problem-solving, and coping capacities.
- Assist individual and family needs, prioritizing services based on the resident’s choice and best interest.
- Provide case management, including goal setting, progress tracking, and referrals to community services.
- Arrange, coordinate, and monitor services for residents and their children, ensuring individualized and holistic support.
- Facilitate individual, family, and group sessions or arrange for outside professionals to provide workshops and specialized support based on professional recommendation and resident desires.
- Assist residents in securing permanent housing, including maintaining a list of housing and rental resources.
- Provide follow-along services after exit to support the transition out of the program, as authorized by the Director
- Act as the first point of contact for residents regarding grievances and ensure concerns are addressed appropriately.
Administrative Duties
- Oversee the preparation of apartments between departing and arriving residents, ensuring safe and well-maintained living conditions.
- Monitor the ongoing apartment needs and tracking of participation in programming from residents.
- Maintain accurate and timely resident records, including case notes, service plans, and appropriate documentation.
- Ensure compliance regarding resident privacy, security, and confidentiality policies in accordance with Federal and State law and grant requirements.
- Supervise volunteers and interns as needed.
Community Engagement & Collaboration
- Develop and maintain relationships with local service providers to provide referral options for residents
- Stay informed about community resources and financial assistance programs available to survivors of domestic violence, including the wide range of needs associated with experiencing domestic violence.
- Work collaboratively with other staff to provide a holistic approach for residents.
- Assist the Director in obtaining donations for residents and distributing them appropriately.
- Plan and coordinate events, holiday celebrations, birthday recognitions, and children’s programming for residents when appropriate.
- Attend local housing assistance and domestic violence-related meetings as directed by the Director.
Knowledge, Skills and Abilities Required
- Bachelor’s degree in Social Work, Human Services, or a related field (Master’s preferred) OR at least four (4) years of experience working with survivors of domestic violence or at-risk populations.
- Supervisory experience preferred.
- Strong knowledge of trauma-informed care best practices.
- Familiarity with tenant/landlord issues and housing-related resources preferred (specifically within Racine County, WI).
- Excellent written and verbal communication skills.
- Strong problem-solving and conflict resolutions skills.
- Ability to work effectively with diverse populations and cultural and societal relevance to issues.
- High level of professionalism, confidentiality, flexibility, and accountability.
- The ability to show empathy and understanding.
- Ability to maintain composure in high-stress situations when they arise.
Physical/Mental Requirements
- Ability to ascent and descend stairs using feet and legs or hands and arms.
- Ability to effectively communicate information in both spoken and written English form.
- Ability to recognize and respond to problems, utilizing deductive and inductive reasoning skills.

- The ability to ask for help and seek support from supervisor or other staff when questions or issues arise outside of your abilities or scope of knowledge.

Working Conditions
Pleasant office environment. Interruptions may be frequent or rare, depending on day and time. Work schedule may involve irregular work hours due to emergent situations, events, or staffing needs.
Job Type: Full-time
Pay: $22.00 - $25.00 per hour
Expected hours: 32 – 40 per week

- Monday to Friday
- Weekends as needed
