Job Purpose:
As a Nurse (RN/LPN), the Navigation Specialist is responsible for coordinating internal and external patient needs including transports, scheduling, and internal supplemental rounds. This role will serve our patients and families with love and respect. The Navigation Specialist will remain flexible and responsive when changes occur in patient activity and workload to support team while maintaining a strong sense of patient and family focus. To be safe, honest, and committed to bettering the lives of those we serve and provide a positive and loving space for those we encounter. Performs safe and efficient transferring and escorting of patients to and from designated areas. May require patient lifting. |

Responsible for coordinating the health care needs of the patient. Connects patients and families to resources and support systems, assists with interaction between healthcare providers, performs supplemental internal rounds, schedules appointments, streamlines paperwork and acts as a patient advocate. |

Qualifications:
Education: |
Completion of diploma Nursing Program (LPN or RN) |
Experience: |
1-Year experience as RN or LPN is required |
License/Certification/Credential: |
RN/LPN: Current LPN or RN license in state of Oklahoma. CPR certification within 1 year of employment. Valid driver’s license. Current in check off for med pass and sedation. |
