Description
Under the direction of the Director of Nursing, oversees residents and staff for half of the building. Is a resource person for Charge Nurse, CMA’s, and CNA’s
- Assist with new admissions; Originate initial care plan upon admission and updates quarterly when assessments are due
- Reviews nursing checklists to ensure completion and compliance.
- Assists nurses with education if check list is not completed correctly
- Assists the MDS coordinator with information for quarterly assessments
- Updates the Matrix three times per week
- Assists in any area on the units as needed
- Conferences with families as needed
- Assist with provider rounds
- Draw lab and help with tele-med appointments
- Psychotropic medication monitoring
- Monitoring of weights, skin integrity, ensuring completion of assessments
- Dietary consults; Review consultant recommendations; Request from PCP as needed
- Pharmacy Consultant: Review consultant recommendations; Request from PCP
- Training of new Nurses
- Attend Medicare; Care Plans; Behavior Team Meetings; QAPI; etc.
- Follow up on any requested changes
- Perform staff education as needed
- Perform med cart audits weekly
- On-call at least one weekend/ month and as needed
- All other duties as assigned.
Qualifications
- Must have a valid Kansas LPN or RN license
- Knowledge of Long Term and Skilled Care
- Effective Time Management
