Job Title: Case Manager, Erie County Independent Living Program
Classification: Regular Full Time
Department: Erie County Independent Living Program
Immediate Supervisor: ILP Supervisor
Job Summary:
Responsible for overseeing the case load of young adults ranging in age from 14 to 23 years. This position will work as a team to assist the youth in learning the necessary skills that will lead them to become independent and self sufficient.
Essential Functions of the Position:
- Provide the necessary skills for youth to prepare them for independency and self sufficiency.
- Participate in a team approach to effectively educate the youth in life skills.
- Complete initial and subsequent assessment for referred cases.
- Aid in the development and documentation of a Case Plan for each youth while receiving services.
- Provide guidance, counseling, and independency preparation for youth.
- Aid clients in recognizing personal short term and long term goals and in determining suitable successive goals.
- Provide education related to independency and self sufficiency in individual and group formats.
- Research and utilize community resources that support aspects of independency and self sufficiency.
- Provide case management and coordination of youth pertaining to program.
- Oversee and coordinate referrals and subsequent communication through. community resources for the benefit of each youth.
- Provide coordination in youth cases when multiple agencies/services are involved.
- Professionally represent the program in meetings, public forums, and court appearances, as required.
- Provide transportation for youth to appointments, job interviews, school visits, and other related activities.
Professional Responsibilities:
- Establish and maintain professional relationships with all agency personnel and outside agencies (OCY, Family Services, schools, etc.).
- Act as a positive role model to youth by maintaining a professional demeanor and ethical manner at all times.
Required Knowledge, Skills, and Abilities:
- Possess effective communication skills, both verbal and written. This includes the ability to converse with residents and families of various socioeconomic backgrounds in a professional manner.
- Highly developed interpersonal skills, organizational ability, plus working knowledge of young adult development is required.
- Knowledge of services needed and available for older youth in order to prepare for independence.
- Ability to develop relationships within the community to support programs for older youth.
Required Education, Experience, and Clearances:
- Bachelor’s degree in human services or related field is required.
- Must be at least 21 years of age.
- Must possess a valid drivers license.
- FBI, Child Abuse History, PA State Police, and Driver Clearances. Other clearances may be required dependent on residency status.
