The role of the Social Worker is to serve as resident advocate, supporting each resident’s rights in the facility and assisting staff, residents, and their families with interpretation of those rights.

The role of the Social Worker is to serve as resident advocate, supporting each resident’s rights in the facility and assisting staff, residents, and their families with interpretation of those rights.
- Social Worker will facilitate the adjustment of residents to the Facility and to the physical and psychological limitations which necessitated or precipitated their placement in the Facility.
- Social Worker will assist residents and their families in understanding and gaining greater acceptance of their limitations and
- encourage residents to engage their strengths.
- Social Worker will enhance the staff’s understanding of residents’ psychosocial needs and motivations, and serve as liaison between the residents, their families, and the facility staff, as necessary and appropriate.
- Visit and interview residents following their admission to the nursing facility, in order to assist newly admitted residents to overcome the trauma of change in environment and to facilitate their adjustment to residence in a skilled nursing facility.
- Social Worker will compile, prior to and after admission, written records of pertinent information obtained from referral sources, appropriate social agencies, the resident, and/or family members.
- Social Worker will prepare the resident’s social/psychosocial history, the psychosocial assessment, and the Social Service Care Plan, within 14 days after admission.
- Social Worker will assess discharge potential and intentions of each resident, and to specify any plans for discharge from the facility on the resident’s written care plan in the medical record.
- Assist the facility staff in understanding the significance of social, psychological, and economic factors in the resident’s life, and to help staff understand the implications of such factors on the resident’s life, behavior, and his/her ability to attain an optimal level of functioning.
- Assist the resident and his/her family in understanding and accepting the resident’s strengths and limitations; assist the resident and his/her family in understanding the plan of care so that they may be optimally supportive of that plan of care and may participate in revision of plan of care as necessary and appropriate.
- Serve as resident advocate by assisting residents in interpreting and implementing their rights and responsibilities in the facility. Encourage residents to voice grievances and/or recommendations as desired and participate, with administrative, medical, nursing, and other personnel in seeking resolution to concerns voiced.
- Serve as liaison between the resident, the psychologist consultant, and the Interdisciplinary Team as necessary and appropriate.
- Review and update psychosocial assessment, care plan, and discharge plan approximately every ninety (90) days, or more often as necessary, and to record such updated assessments and plans in the residents’ medical record.
- Minimum of a Bachelor’s degree in Social Work or a related field
- Minimum of two years of experience in the social services field, preferably in a nursing facility, hospital or related agency, preffered
- Pleasant personality and ability to work with residents, families, and staff from various departments.
- Proficient in Microsoft Office.
- Excellent communication and organization skills.
- Ability to work well under pressure and be flexible.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents
- Work well under pressure & willing to work harmoniously with all staff
