Job Title: Program Coordinator (Designated Coordinator)
Location: Minneapolis
Reports To: Designated Manager and/or Business Director
Job Summary:
The Program Coordinator (Designated Coordinator (DC)) is responsible for ensuring the quality and compliance of services provided under Minnesota’s 245D Home and Community-Based Services and Community First Services and Supports (CFSS)/Personal Care Assistance (PCA) programs. This position oversees service delivery, staff training, and compliance with state regulations to support individuals with disabilities and other care needs.
Essential Duties and Responsibilities:
- Ensure that all services comply with MN Statutes, Chapter 245D, CFSS/PCA requirements, and Minnesota Department of Human Services (DHS) regulations.
- Develop, implement, and monitor person-centered support plans, CSSPs (Coordinated Service and Support Plans)/ Support Plan Addendum (SPA) and service outcomes.
- Conduct assessments and reassessments to determine individuals’ service needs.
- Conduct periodic service reviews and ensure documentation meets regulatory standards.
- Conduct outreach activities via email, telephone calls, and/or distribution of promotional materials.
- Supervise and support Direct Support Professionals (DSPs)/Caregivers, PCA/CFSS staff.
- Train staff on 245D policies, CFSS/PCA procedures, clients’ approved services, de-escalation techniques, positive support, and emergency protocols.
- Ensure staff compliance with training, background checks, and certification requirements.
- Act as a liaison between individuals served, families, case managers, caregivers, and others in the support network
- Address client concerns, resolve service issues, and advocate client needs.
- Ensure services align with each individual’s personal preferences and needs.
- Maintain accurate records, including service notes, progress reports, incident reports, and health-related documentation.
- Submit reports and compliance audits to DHS and licensing bodies as required.
- Monitor service authorizations to ensure accuracy and compliance.
- Develop and implement emergency response procedures.
- Provide guidance and intervention during crises.
- Address and resolve issues related to client care, staffing, and service compliance.
· Ensure corrective actions are implemented and documented effectively.
Requirements:
Education & Experience:
· Bachelor’s degree in a human services-related field (Social Work, Psychology, Special Education, etc.) and one year of full-time work experience providing direct care to persons with disabilities or individuals with health conditions OR
· Associate degree in a human services field and two years of full-time work experience providing direct care
Preferred Qualifications
· Experience in case management or similar role
Skills & Competencies:
- Strong knowledge of MN 245D licensing requirements and CFSS/PCA service regulations.
- Ability to train, supervise, and support direct care staff and caregivers.
- Excellent documentation and compliance management skills.
- Strong interpersonal and problem-solving abilities.
- Ability to work independently and manage multiple priorities.
Work Environment & Schedule:
- Part-time with full-time opportunity in the future depending on performance and organization’s need.
- On-call responsibilities may be required for emergency response.
- Work is performed in the office, client homes, and community-based settings.
