Better Health Together (BHT) has been leveraging experience, knowledge, and resources to coordinate action and address local and regional health inequities across Ferry, Stevens, Pend Oreille, Spokane, Lincoln and Adams counties and the Reservations of the Kalispel Tribe of Indians, the Spokane Tribe of Indians, and the Confederated Tribes of the Colville reservation since 2014.

About the Community Care Hub

BHT is now working across health care and social service delivery systems to create a coordinated Community Care Hub (CCH) so that all members of our community have access to the resources they need to thrive and be healthy. We were founded on the principle that when we step back and let the local community lead, we find the most equitable and sustainable solutions to some of our most complex problems.

About the Position

The Referral Coordinator will report to the Social Care Network (SCN) Manager and will work closely with the Care Coordination team to carry out key responsibilities. The Referral Coordinator works with the SCN Manager to administer referrals that come through the Hub. They will manage incoming and outgoing referrals within Client Coordination System (CCS) and ensure people with identified needs are connected to the appropriate SCN partner and resources. They will support the program in ongoing collecting, monitoring, cleaning, and reporting of data and continuous quality improvement to support SCN partner capacity, and feedback from SCN Partners and their workforce to determine appropriate referral placement and specific, measurable, achievable, relevant, and time-based (SMART) assignments.

Impactful Problem Solver Highly Organized

  • Coordinate the overall referral processes within the hub, ensuring smooth and efficient operation of referral systems.
  • Maintain protocols for receiving, processing, and following up on referrals to ensure timely and appropriate care coordination, e.g. SCN partner capacity and appropriate referral types; Baseline referral rates and targets.
  • Provide training and onboarding for new referral partners, ensuring they understand their roles and responsibilities within the referral network
  • Provide training and resources to new referral partners to equip them with the necessary tools and knowledge for effective participation in the referral system.
  • Regularly evaluate the performance of referral partners to ensure they meet the required standards and protocols.
  • Address any issues that arise in the referral process, implementing solutions to improve efficiency and effectiveness.
  • Maintain ongoing communication with all referral partners to keep them informed of any changes in procedures, policies, or operational needs.
  • Continuously seek ways to improve the referral processes and partnership arrangements.
  • Gather feedback from referral partners and clients to identify areas for enhancement and implement changes to optimize the referral system.
  • Support opportunities to enhance resource and referral opportunities in the region
  • Utilize effectively BHT technology systems for referral monitoring, care coordination and program monitoring of the Network (e.g. Care Coordination System (TMP), Asana, Fluxx, SharePoint)
  • Collect and Enter Data: Gather data from various sources, such as surveys, databases, spreadsheets, or directly from users/clients.
  • Monitor Data Integrity: Check for completeness, accuracy, and consistency in datasets, performing quality control checks to identify and correct errors.
  • Data Cleaning: Identify and remove duplicates, errors, and inconsistencies from datasets to ensure clean, usable data.
  • Maintain Data Dictionaries: Create and update data dictionaries or metadata to explain the meaning, format, and structure of the data.
  • Document Procedures: Maintain documentation on data entry methods, data sources, and any standard operating procedures.
  • Report Generation: Create periodic or ad hoc reports for stakeholders, summarizing data trends, performance, or other insights based on predefined metrics.
  • Support Projects: Coordinate data collection, track milestones, and ensure data is aligned with the project’s goals.
  • Evaluate potential pitfalls and bring them to their supervisor’s attention;
  • Create workflows and process for the following:
  • SCN partner capacity and appropriate referral types;
  • Baseline referral rates and targets

Qualifications

  • Solid organizational and time-management skills
  • Experience with accounts payable
  • Highly competent and confident with technology
  • Experience with handling/managing and reviewing sensitive documents
  • Experience with invoice processing and/or managing a budget
  • Great data management skills
  • Proficiency in technology in a productive and efficient way
  • Able to prioritize and multi-task in a fast-paced work environment
  • Excellent interpersonal, verbal, and written communication skills necessary to communicate effectively with internal and external partners
  • Demonstrate attention to detail, accuracy, and precision, which supports but is not limited to grant compliance requirements

Working Conditions

This position requires the ability to:

  • Remain in a stationary position for prolonged period of time;
  • Move/traverse within a multi-level office space, to/from community partner locations, and various meeting spaces;
  • Often move/transport equipment, supplies that may exceed 30 pounds;
  • Constantly operate a computer and other office productivity machinery, such as a calculator, copy machine, and printer; and
  • Communicate frequently, effectively, and accurately with internal and external partners.

Environmental Conditions

Generally, in an office environment with visits to external environments that require traveling to and from various locations within the BHT region of Spokane, Lincoln, Pend Oreille, Stevens, Ferry and Adams County and occasionally to the west side of the state. Post-COVID, BHT has adapted a hybrid work approach, encouraging in-person meetings with team members and community partners. Pay: $69,000.00 - $80,000.00 per year
