Key Responsibilities
- Develop and implement annual, special, and 30-day individual service plans according to written policies and procedures.
- Conduct client ISP's staffing's and complete client ISP's after staffing's according to written policies and procedures.
- Implement client programs, train staff, and provide effective counseling and case management services.
- Establish and maintain community networks and interactions, and provide consultation and input to leadership as requested.
- Bachelor's Degree in a Human Service Field
- 1 year of experience working directly with persons with an intellectual disability or other developmental disability
- Proficient in computer skills and comfortable with Microsoft Office
- Good Oral & Written Communication Skills
