Position Summary:

A member of the Addiction Consult Service to provide non-clinical support to individuals who present in an ED and/or Inpatient at Beverly and Addison Gilbert Hospitals post-overdose. Serve as a recovery guide and role model in the management of recovery and assist the individual to identify and overcome barriers to recovery, connect the individual with recovery support services and encourage hope, optimism and health. Provide support with problem solving and advocacy to assist individuals to meet their recovery goals. Provide education to individuals and family members on the recovery process. As a member of the Addiction Consult Service, this position engages with patients who are high ED utilizers to coordinate with peer networks/natural supports, coordinate referrals to community social services providers, follow up on referrals and contribute to the treatment plan revisions.

Job Description:

  • Essential Role Responsibilities
  • Meet with individuals who present in an ED post-overdose and act as a resource for any individual who presents to the ED seeking services for a substance use disorder.
  • Be knowledgeable about the substance use disorder service system and link the individual to treatment and recovery resources including but not limited to Acute Treatment Services, sober housing, benefits, and Narcan education.
  • Act as an advocate for the needs of the individual
  • Provide education on the signs and symptoms of an overdose, overdose prevention and the use of naloxone to the individual and offer the individual an overdose prevention kit.
  • Provide short-term telephonic (or text) or in-person follow-up support, coaching, and assistance to help the individual access treatment or recovery support services. Obtain the phone number of the individual, if possible, and give the individual his/her contact in order to follow-up the next day.
  • Conduct a warm handoff of the individual to appropriate treatment or recovery services including a Community Support Program (CSP) to encourage continued treatment and recovery support in the community
  • Demonstrate an understanding and belief in the recovery process and Recovery Oriented Systems of Care Model
  • Knowledge of basic crisis intervention, Motivational interviewing, and case management techniques preferred.
  • Demonstrates good boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from a substance use disorder.
  • Demonstrates the ability to evaluate what is needed by each individual and adjust approach accordingly.
  • Have in-depth knowledge of local resources.
  • Provides temporary assistance with transportation as needed to essential behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation resources, PT-1 forms, etc.).
  • Refer the individual to Intensive Case Management Program (ICM) if within their health insurance benefit.
  • Complete training provided by agency and participates in ongoing supervision by a staff person who is also trained and as a Recovery Coach and has a greater length of experience than the supervisee as a Recovery Coach;
  • Ensures that all written and billing documentation is complete in a timely manner.
  • Manages CHART Tasks by patient and updates care manager tool (for CHART work only)
  • Other related duties as assigned.

Education/Experience:        

  • 18 years or older and high school diploma required; bachelor degree preferred.
  • Driver’s license and car required.
  • Be a Peer with lived addiction experience.
  • Have at least two years of current, sustained recovery without reoccurrence.
  • Complete the Bureau of Substance Abuse Services (BSAS) Recovery Coach Academy or the Connecticut Community Addiction Recovery (CCAR) training prior to working with individuals in the ED and within one month of hire. (for ESP work only)
