Summary

Under direction, performs five key functions, comprised of assessment, planning, linking, monitoring, and advocacy. The case manager is the centralized recipient of critical information, as it pertains to case management, who will disseminate case management information to all team members in a timely manner.

The Veterans Court Case Manager determines applicants' eligibility for a variety of social services available and / or provides other social services related to Veterans Court crisis intervention and case management. Interviews clients, collects and completes required eligibility documentation, determines eligibility for social services assistance and monitors client progress and usage of assistance. Refers clients to clinical and social service provider resources, and performs related duties as required.

Duties and Responsibilities

  • Interviews clients, collects and completes required documentation and determines eligibility for social services assistance based upon a varied of service regulations and procedures
  • Conducts assessments of client needs, wants, strengths, and resources and makes initial determination of the client's psychosocial situation
  • Collaborates with the client and the Veterans Treatment Court team and develops casework / service plans by defining clients' goals, developing strategies for each goal, identifying who is responsible for each action in the strategy, and establishing timeframes
  • Ensures that documents and records needed for Veterans Court staffing are gathered and ready for the staffing
  • Links the client to needed services and supports provided by social service agencies, organizations, such as, but not limited to, the Veterans Administration, Veterans Health providers, Veterans Support groups and vendors to provide appropriate services, information to clients
  • Monitors the use of services, intensity level of services and determines if the client is benefiting from the service, by maintaining ongoing communication with services and supports, and conducting ongoing assessments of the client's progress through follow-up sessions
  • Advocates on behalf of the client by helping the client access services for which he or she is eligible through : education of service providers, persuasive communication, negotiation, and use of policy and rights-protecting laws and rules
  • Plans, organizes, and participates in special needs projects including outreach efforts, public speaking, and special activities to clients and the Bexar County community
  • May perform related duties as required

Minimum Qualifications

  • Graduation from an accredited college or university with a Bachelor's Degree in Psychology, Counseling, or Social Work and three (3) years of experience in social services related work; or an equivalent combination of education and experience
  • Knowledge of mental health treatment protocols, social service policies / procedures and of local social services, organizations, programs, and resources available; knowledge of Federal, state and local programs laws and requirements; knowledge of the legal system as it relates to the mentally ill; knowledge of computer equipment to include word processing, spreadsheets, databases and a variety of software packages
  • Skill in providing assistance to clients and referring clients to appropriate social services organizations and programs; skill in interviewing techniques; skill in problem solving and decision making
  • Ability to deal with persons experiencing mental health issues including Post Traumatic Stress Syndrome and Traumatic Brain Injury; ability to communicate effectively with hostile, abusive or irrational individuals; ability to work effectively under high-pressure circumstances; ability to communicate effectively, both verbally and in writing; ability to communicate effectively with diverse groups of individuals utilizing tact and diplomacy; ability to operate personal computer and basic office equipment; ability to establish and maintain effective working relationships with clients and / or families, co-workers, County employees, and community outreach and other organizations, clinical and other social service providers
  • Must secure and maintain a favorable background investigation and clearance
  • Must clear a pre-employment physical and a pre-employment drug screen test
  • May be required to work more than 40 hours during the workweek
  • Working Conditions and Physical Demands :

  • Constantly working in indoor conditions with occasional exposure to hostile or violent individuals
  • Occasionally moves items weighing up to 10-15 pounds
  • Occasionally moves about the office
  • Frequently communicates with others; must be able to exchange accurate information
  • Constantly operates a computer and other office productivity machinery
  • Positions self to perform essential functions
  • Must be able to remain in a stationary position
