Job Summary
We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team. This role involves assessing, diagnosing, and treating patients with various types of wounds in both clinical and home care settings. The Wound Care Nurse Practitioner will work collaboratively with other healthcare professionals to develop comprehensive care plans that promote healing and improve patient outcomes.
Responsibilities
- Conduct thorough assessments of patients with wounds, including history-taking and physical examinations.
- Develop and implement individualized treatment plans based on best practices for wound care.
- Perform wound debridement, dressing changes, and other necessary procedures to promote healing.
- Educate patients and their families on wound care management, discharge planning, and prevention strategies.
- Collaborate with physicians, nurses, and other healthcare providers to ensure comprehensive patient care.
- Maintain accurate documentation of patient assessments, treatments, and progress in compliance with HIPAA regulations.
- Stay updated on the latest advancements in wound care techniques and technologies.
- Provide support in clinic settings as well as during home care visits when necessary.
Qualifications
- Master’s degree in Nursing or related field with a focus on wound care preferred.
- Current Nurse Practitioner licensure in the state of practice.
- Certification in Wound Care (e.g., CWCN or CWS) is highly desirable.
- Experience in clinic settings, hospital environments, or home care is preferred.
- Proficient in basic life support techniques and catheterization skills.
- Strong understanding of discharge planning processes for effective patient transitions.
- Excellent communication skills with the ability to educate patients effectively.
- Knowledge of HIPAA regulations to ensure patient confidentiality and data protection.
