Requirements:
- Certified Nursing Assistant (CNA)
- Valid Driver's License with personal Vehicle with proof of auto insurance
- 3 years Home Care or Senior Care experience required.
- Must be 18 years of age or older. High school or GED required.
Schedule Availability:
- Must be able to work 5 day per week
Lead Caregiver Responsibilities:
- Provide personal care services, including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, eating, and mobility.
- Assist clients with medication reminders, light housekeeping, meal preparation, and companionship.
- Monitor client health conditions, report any significant changes to the Staffing Manager, and maintain accurate client records.
- Ensure the safety and well-being of clients at all times, following established care plans and guidelines.
- Maintain accurate and timely documentation of client interactions and care notes in the AxisCare system.
- Maintain continuity of care by being flexible and dependable, including being available for urgent shift coverage, working short notice for callouts, vacations, or emergencies. Adapt to changing schedules as needed to meet the care needs of clients.
- Maintain clear and effective communication with Staffing Manager. Promptly reporting any issues, concerns, or updates.
- Must demonstrate excellent phone etiquette and communicate professionally with the management team, caregivers, and clients/families.
- Transport clients to and from appointments or other locations as needed. Must have a reliable personal vehicle and be willing to travel up to 30 miles, depending on client needs.
- Participate in a rotating on-call weekend schedule, covering a 24/7 phone line to handle caregivers or client emergencies, call-outs, or issues. This will include troubleshooting staffing concerns and ensuring care continuity.
- Must be available during the agreed-upon consistent schedule (see Appendix A) and remain reachable to answer phone calls from management at all hours during this schedule.
- Proficiently manage caregiver schedules, ensuring proper staffing coverage. Experience with electronic medical records (EMR) systems is required. The ideal candidate should also be excited to learn and adapt to new technologies as the company evolves.
- Visit new clients during their first shift to ensure their needs are being met and that the assigned caregiver is providing the required care.
- Required to provide a two-week notice if stepping down to a caregiver role or leaving the company.
