Community Case Manager
Community Case Manager – Connect People to the Care They Deserve
Are you passionate about helping individuals access critical mental health support and community resources? BHcare is seeking a compassionate and motivated Community Case Manager to join our Adult Clinic team. This is a dynamic role focused on outreach, engagement, and building meaningful connections to improve client outcomes.
What You’ll Do:
• Provide outreach and limited transportation to help clients build independence in navigating their community
• Support IOP, Crisis, ACT, and outpatient teams in client engagement, follow-up, and retention
• Assist clients with accessing medical and psychiatric care following crisis events or as part of their ongoing treatment
• Offer education and support to families, significant others, and community members to promote client stabilization and recovery
• Facilitate or co-facilitate psychoeducational and orientation groups as needed
• Help clients understand and maintain public benefits and health coverage
• Connect clients to community-based resources like AA/NA, soup kitchens, diaper banks, and clothing banks
• Provide case management support for medication-only clients, including documentation, outreach, and discharge coordination
• Participate in the development and review of integrated service plans tailored to client needs
• Conduct breathalyzer or urine screenings when required
• Maintain accurate documentation in the electronic health records system (EHR)
What You Bring:
• Bachelor’s degree required (social work preferred)
• Knowledge of Psychiatric Rehabilitation practices a plus
• Valid driver’s license with reliable transportation and acceptable driving record
• Proficiency in Microsoft 365 and ability to navigate EHR systems
• Strong interpersonal and communication skills, with a commitment to equity and person-centered care

Qualifications
MINIMUM QUALIFIATIONS
- Bachelors degree required, bachelors degree in social work preferred.
- Knowledge of Psychiatric Rehabilitation is desirable.
- Must have and maintain a valid CT drivers license if relocated to CT as a state resident more than 30 days ago. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organizations auto insurance plan.
- Must be proficient with PCs and Microsoft Office Word & Outlook, and if applicable ability to navigate through electronic health record.
Salary : $23 - $33
