What You’ll Do:
- Provide daily living assistance including hygiene, meal planning, medication protocols, cleaning, and budgeting
- Develop and implement individual recovery plans in coordination with clients, treatment teams, and natural supports
- Facilitate rehabilitation groups and provide interventions to help clients build life skills and maintain stability
- Monitor and document client progress using electronic health records (EHR) and communication logs
- Provide transportation and accompany clients to appointments as needed
- Ensure safety and well-being of clients during all shifts, including crisis response and overnight supervision
- Complete referrals and linkages to mental health, vocational, and community resources
- Use principles of Motivational Interviewing and support clients with co-occurring disorders using the IDDT model
- Maintain medication logs and ensure safe protocols for self-medication
- Perform routine cleaning and third-shift documentation as required

- Education/Experience: Two years of college OR high school diploma/equivalent with 2 years of direct care experience
- Skills: Strong communication, basic computer proficiency (Microsoft 365), EHR navigation
- Mindset: Empathy, patience, organization, and a passion for helping others thrive
- Licenses: Valid driver’s license with insurable driving record
- Bonus: Public Passenger License (or willingness to obtain – not required for overnight shifts)

Qualifications
Qualifications:
- Two years of college education or High School Diploma or Equivalent and 2 years direct care work experience.
- Must have working knowledge of computer applications, and if applicable ability to navigate through electronic health record
- Must have and maintain a valid CT drivers license if relocated to CT as a state resident more than 30 days ago. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organizations auto insurance plan.
- Must possess or have the ability to obtain and maintain a Public Passenger License. (Staff on the overnight shift may be excluded from Public Passenger License requirement).
- Ability to obtain and maintain CPR and First Aid certification
