Job Description:
- Registered nurse providing diabetes education services to patients and staff and performs a variety of duties and responsibilities related to inpatient and outpatient diabetes care, including community events related to diabetes.
- Includes patient care activities related to diabetes management, prevention of diabetes, clinical outcome data collection and analysis, quality improvement activities, staff education, medical staff education, and community education.
- Delivers diabetes education in accordance with national standards and ADA Recognition requirements through various delivery methods including in-person and telehealth/phone methods to individuals and groups.
- Collaborates as an effective member of the health care team in both settings.
Requirements:
- Bachelor's of Nursing
- Current RN License
- Two years related experience
- Certified Diabetes Care and Education Specialist; Must Pass Certified Diabetes Care and Education Specialist exam within 6 months of NCDBE eligibility, unless extension granted.
