Nurse Navigator Lexington, KY
Full Time, 1st Shift

Description:
- Serves as a clinical resource, patient advocate, and system navigator for cancer care and serves as a liaison between the organization, patients, families, and the community for cancer groups
- Provides written and verbal breast cancer information related to the patient’s diagnosis, pathology plan of treatment, and survival plan of care
- Facilitates information gathering and analysis for patients to make informed decisions
- Assesses patient’s educational and physio/psychosocial needs and barriers to care
- Individualizes the care of breast cancer patients throughout the continuum of care
- Collaborates with multidisciplinary team members to facilitate timely access and continuity of care across various care settings
- Works with healthcare providers in maintaining concordance with evidenced-based care and with established and adopted guidelines
- Enhances self-development on a continual basis by maintaining current knowledge in clinical education, reviewing professional literature and participating in professional organizations
Requirements:
- Bachelor of Science in Nursing (BSN)
- Minimum of two years of relevant work experience
- Current KY state license as a registered nurse
- Basic life support
- Oncology nursing experience preferred but not required
- Must obtain Certification in Oncology Nursing (OCN, AOCN, AOCNS or AOCNP) within 1 year of hire
