Job Description:
Baptist Health is looking for a Home Care Physical Therapist to join our team in New Albany, IN
The physical therapist evaluates the patient's comprehensive health care needs in the home environment and implements an appropriate plan of care according to those needs utilizing the client’s level of function by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests, community resource and the patient’s support system.
Minimum Education, Training, and Experience Required
• Advanced Degree PT
• Professional license PT
• BLS Healthcare Provider
• One year of experience as an Physical Therapist
• Home Health experience preferred
