As a Social Worker at Bibb Medical Center, you will play a crucial role in enhancing the well-being of patients/residents and their families by providing comprehensive support and resources. Essential Duties and Responsibilities:
The Social Worker reports to the Director of Social Services and works in conjunction with Nursing Home and hospital clinical staff/care team, business office and the activities department to ensure overall health and well-being is being cared for. Social services focus on the psychosocial, spiritual, emotional needs of the patients during times of admission, transition, grief, and general daily living and understands the importance of mental health in relation to physical health. Social services may also provide support and interventions for patients’ families.
Other duties also include:
- Assisting residents and/or families in admission paperwork.
- Completing social history and inventory list.
- Developing psychosocial care plans, goals, and interventions.
- Discharge planning: home safety assessments, DME, home health, outpatient therapy, follow-up doctor appointments, transportation, and family support.
- Coordinate with interdisciplinary team to ensure needs are being met.
- Identify and evaluate personal, emotional, and environmental concerns of patients and coordinate interventions.
- Complete routine MDS assessments and other mental health screenings.
- Record continuing progress notes on patient’s social needs and interventions in place.
- Assist patients and families with Advanced Care Planning/DNR forms and review periodically.
- Review/record complaints and grievances and follow-up as appropriate.
- Keeps record of residents at risk for elopement and smokers and evaluate periodically.
- Link patients and families with community resources.
- Evaluates for and coordinates psychiatric services in conjunction with clinical staff and social services director.
- Coordinates care conferences with interdisciplinary team, patients and family members to assist in better understanding resident care or concerns.
- Attends required in-services, care plan meetings, team meetings.
- Participates in quality assurance measures.
- Assist in lost item reports and finding/replacing items in timely manner.
- May assist with/execute weekly patient shopping.
- May assist residents in room organization and maintaining tidiness.
- Performs other various duties as assigned.

Education:
- Bachelor's degree required. Social work degree preferred.
