Location: Must be local to Tallahassee, Florida.
Job Summary:
The Clinical Hospital Liaison is a critical role that responds to direct requests for assistance, referrals, and education within a hospital setting. The Hospital Liaison provides comprehensive support and direction to facility staff in delivering nursing services to patients and their families, coordinates case management of patient services and medical care, and works within an interdisciplinary care setting to assist and support patients and families through the dying process.
What you will do:
(Includes but, not limited to)
- Respond to direct requests for assistance, referrals, and education within assigned hospitals
- Utilize informal and impromptu opportunities to educate, support, and develop solution driven resolutions that support positive partnerships with hospital resources and contacts.
- Provide support and direction to facility staff in delivering nursing services to patients and families that may benefit from hospice services.
- Coordinate case management of patient services and medical care for those who may qualify for hospice services.
- Work in an interdisciplinary care setting to teach, assist, and support patients and families through the dying process.
- Participate with and provide feedback to the Team Manager and other department personnel in the development and implementation of plans for ongoing hospice focused education and outreach.
- Works closely with BBH staff and providers to coordinate admission of GIP patients to our inpatient units.
- Partners with hospital on discharge planning process including transportation, medications, DME, etc.
- Conducts daily visits to case managers within their assigned hospital to inquire about potential referrals or business opportunities.
- Participates in cross training with the professional relations liaisons in the field.
- Hosts quarterly lunch and learns with case managers at assigned hospitals.
- Utilizes the Customer Relationship Management (CRM)tool for documentation of efforts, contacts and outcomes.
- Other duties as assigned.
Education and Experience:
- Bachelor's degree in healthcare or related field.
- Current State of Florida or eligible Compact RN, LPN or MSW license required.
- Experience in hospice or palliative care preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively in an interdisciplinary team setting.
- Compassionate and supportive approach to patient and family care.
- Ability to work assigned weekends and on-call shifts.

- Excellent verbal and written communication skills.
- Excellent interpersonal, negotiation, and conflict resolution skills.
- Excellent organizational skills and attention to detail.
- Strong analytical and problem-solving skills.
- Ability to effectively manage competing priorities.

- Proficient with Microsoft Office Suite or related software.
- Knowledge of fundraising skills.
- Ability to meet deadlines and work under pressure.
- Excellent people skills and ability to work effectively with providers, clinicians and others within the healthcare setting.
- Function as part of a team member while working independently.
- Availability to work nights and weekends as required.
