Description
We are hiring a compassionate and professional LPN to join our IPU team! The LPN will be responsible for providing outstanding patient care based on the nursing process, planning, implementation, and monitoring of IPU patients in accordance with the physician's order and under the supervision of the Registered Nurse.
Schedule: Full-time, Nightshift

Qualifications
Duties and Responsibilities:
(Includes, but not limited to):

Assist RN in providing care to patients and families.

Monitor and evaluate medication needs and administration of patients including symptom management and equipment needs.

Documents patient visits.

Communicates progress and needs of patients and families to other interdisciplinary team [IDT] members including physician.

Assess patient safety and provide education for safety issues to patients and families.
-
Refer patients to other disciplines as needed.
-
Satisfactorily complete the competency requirements for this position.
Clinical
-
Under the supervision of a RN, the LPN provides palliative nursing care to patients and families, incorporating psychosocial, cultural, physical, spiritual, and biological components and appropriate nursing intervention as identified in the patient/family plan of care, developed by the IDT.

Demonstrates clinical skills regarding application of Hospice Clinical and Administrative policy and procedures such as: pain medication titration, symptom management, and patient/family relationships.

Observes and reports signs of adverse reactions, response to treatments and results of dressing changes.

Consults and collaborates with other professionals to promote quality and continuity of patient/family care.

Accepts responsibility and is accountable for nursing actions; demonstrates an understanding of legal consequences of nursing actions; requests supervision when needed; performs within the guidelines established by the Florida Nurse Practice Act.
Administrative

Nursing documentation displays accuracy, timeliness, legibility, professional terminology and awareness of legal implications when nursing care is delivered

Presents concise, pertinent oral and written patient information to team; respects input from all disciplines

Incorporates behaviors supportive of BBH internal mission statement

Performs within the framework of the philosophies, policies and procedures of Hospice
Communication

Communicates accurately and completely to staff members, patients, families and supervisors; utilizes positive approaches when working with other staff members.
Completes all other duties as assigned.
Requirements:
- Must have an active Florida LPN License
- 1 year experience in a medical/surgical inpatient unit
- BLS Required
- Must maintain an active Florida Driver's License
- Hospice experience preferred
- Compassionate with sound judgment
- Excellent communication and organizational skills
- Proficient in Microsoft Office and medical software systems (for example: Matrix Homecare)
- Ability to handle stress and long shifts
