Position Overview : We are seeking a dynamic Social Worker, to assess the phychosocial needs of residents, develop personalized care plans to adress those needs, providing emotional support and counseling. Advocating and coordinating with other healthcare professionals to ensure residents receive comprehansive care, all while facilitaing a smooth transition into and with in the nursing home environment; essentially acting as a bridge between residents, families, and the healthcare team to maintain quality of life and well-being.
Key Responsibilities :
Conducting evaluations of resident' emotional, social, and physhologicalneeds upon admission and regularly throughout their stay.
Collaborating with the healthcare team to create indivudualized care plans that address the resients's unique needs, including coping mechanisms, support syst;ems, and desired activities.
Providing emotional support and counseling to residents and their families regarding issues like grief, loss, adjustment to the nursing home, and decision-making realated to care.
Facilitating open communication with families, keeping them informed about the resident progress,k addressing concerns and providing support during difficult transitions..
Acting as the resident advocate by ensuring their rights are protected, addressing complaintd, and navigating complex healthcare systems to aaccess necessary services.
Coordinating discharge plans with residents, families, and community resources to ensure a smooth transitionupon leaving the facility.
Identifying and connecting residents and families with appropriate community resources like fiancial assistance, support groups, and home healthcare servaices.
Provideing immediate support in crisis situations such as sudden changes in ehalth status or emotional distress.
- Maintaining accurate and detailed records of residents assessments, interventions, and progress.
Qualifications : The ideal candidate for this position will have :
Strong communication and interpersonal skills to build rapport with residents, families, and staff
Ability to conduct psychosocial assessments and develop appropriate interventions.
Knowledge of aging and related health issues, including end-of-lefe care.
Understanding of ethical and legal considerations in healthcare settings.
Experience in crisis intervention and conflict resolution.
Ability to work independently and as part of a team.
