Job Overview
We are seeking a compassionate and dedicated Registered Nurse - to join our team. You will be responsible for assessing patient needs, developing care plans, and collaborating with other healthcare professionals to deliver comprehensive care. Duties
- Conduct thorough assessments of patients' health status and needs in their home environment.
- Develop, implement, and evaluate individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed by physicians.
- Provide education and support to patients and their families regarding health management and treatment options.
- Perform skilled nursing procedures such as acute pain management, airway management, specimen collection processing, and other clinical tasks as required.
- Coordinate with interdisciplinary teams to ensure comprehensive patient care.
- Document all patient interactions and care provided accurately in electronic health records.
- Participate in case management activities to facilitate continuity of care.
- Engage in ongoing professional development and training related to hospice palliative medicine, occupational health, and travel nursing as needed.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health or similar settings preferred.
- Knowledge of MDS (Minimum Data Set) processes is a plus.
- Strong skills in acute pain management, airway management, outpatient care, and specimen collection processing.
- Excellent communication skills for effective patient education and collaboration with healthcare teams.
- Ability to work independently while managing multiple priorities effectively.
- Willingness to travel to patients' homes within the designated service area.
- Commitment to providing compassionate care with a focus on patient-centered outcomes.

Pay: $35.00 - $37.00 per hour
