Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The Hospice Social Worker helps patients and their families cope with the emotional, practical, and social challenges of serious illness. They advocate for patients' needs, connect patients with community resources, and help patients and families make decisions about end-of-life care.
Responsibilities
- Assess needs: Evaluate patients' psychosocial symptoms, strengths, and support networks
- Provide counseling: Help patients and families cope with grief, anxiety, and other emotions
- Help with decision making: Assist patients and families with end-of-life decisions and goal setting
- Connect patients with resources: Help patients and families access community resources, mental health treatment, and other services
- Promote advanced care planning: Help patients and families plan for the future
Hospice Social Workers are part of an interdisciplinary team that works to improve the quality of life for patients and their families.
- Familiarity with geriatrics and aging issues counseling is highly desirable.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
- Strong organizational skills and attention to detail in documentation practices.

Pay: $30.00 - $40.00 per hour
