As an LVN / LPN you will deliver Home Health & Hospice care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and you will effectively interact with patients, and other health team members. Provide service for patients through 9-week episodes, ensures commitment to quality patient care, prepared to travel and have open availability to patients as needed . The best solutions are not always the most obvious; we are looking for outstanding LVN's / LPN's who are passionate on delivering valuable patient care. Qualifications :
- Part-time and full-time available
- Ensure quality and safe delivery of home health nursing services in accordance with State regulations and Billet Health policy.
- Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and / or needs.
- Current license as a LPN / LVN in the state (s) of practice
- Licensed driver with automobile insured in accordance with the state and / or agency requirements
- COVID Vaccine (Exemptions accepted)
- Current TB Test
- CPR or BLS certification
- Arizona Level One Fingerprint Card
