We are an established healthcare continuum care company that is seeking a entrepreneurial and self motivated full-time midlevel provider to assist with the growth of our mobile primary care division. Our clinic focuses on providing our patients with high quality primary care and advanced wound care while incorporating innovative healthcare technology and programs that improve patient outcomes, such as chronic and complex care management, remote patient monitoring and transitional care management. We have the ability to offer our patients a full care continuum model that consists of a mobile primary care provider, home health and hospice care that assists in keeping at risk patients comfortable in their home setting without the need to return to the hospital or skilled facility setting. The ideal candidate for this position must possess servant leadership qualities and work with the leadership team in becoming the golden standard for primary care in the designated target markets.
Job Description:
· Provide high quality patient care in a house call setting (Assisted Living, Group Homes and Patient’s private homes)
· Evaluate patients and provide appropriate medical treatment for various illnesses and injuries
· Document all patient evaluations, treatments, medications and transactions per company policies and procedures
· Perform initial patient screenings and patient history and physical examinations
· Update patient records and check records for accuracy at each patient appointment
· Analyze test data and determine the need for follow-up appointments and further treatment options
· Serve as the resource patients need for ongoing care information, counseling and provide guidance for any patients with critical conditions
· Prescribe, administer and dispense medication in accordance with state and federal statutes
· Market primary care and specialty services to senior living communities while on site providing care to other residents
· Build and grow a strong relationship with senior living community stakeholders and leadership to solidify Link Medical’s position as top provider on campus
· Works collaboratively with office administrator and staff to achieve patient care and quality objectives
· Identify and proactively solve problems
· Knowledge and ability to perform therapeutic interventions such as splinting, suturing, wound care, minor surgical procedures
· Strong interpersonal and written communication skills
· Critical thinking skills utilizing evidence based standards of practice
· Expertise in clinical skills related to acute care, chronic conditions and injuries.

Job Requirements and Skills:
· Two or more years of clinical experience as a nursing practitioner at a brick and mortar practice or in a hospital setting with a related specialty is strongly preferred.
· Valid licensure in the State of Arizona or ability to obtain prior to employment
· Board certification as a Nurse Practitioner in the state of which the applicant is applying
· National board certification as a Nurse Practitioner from an accredited agency
· Active DEA license with prescribing authority
Preferred experience in advanced wound care and skin substitute applications
· Strong interpersonal and written communication skills
· Critical thinking skills utilizing evidence based standards of practice
· Expertise in clinical skills related to acute care, chronic conditions and injuries.
· Thrive in a patient-focused environment
· Ability to work within a team and take direction
· Knowledge and experience with billing and coding is preferred

Compensation: fee per visit, profit sharing opportunity, base salary plus incentives
