The Regional Care Coordinator is responsible for providing support and guidance for care coordination functions within an assigned region of skilled nursing facilities, while maintaining direct care coordination responsibilities for designated facilities. This role manages a direct caseload at facilities as assigned by leadership, while ensuring that residents across all regional facilities receive comprehensive, high-quality care tailored to their individual needs. The Regional Care Coordinator will provide mentorship to the Care Coordinator team within the region, collaborate with healthcare providers, and develop strategies to improve patient outcomes and operational efficiency.Essential Duties and ResponsibilitiesDirect Care Coordination : Serve as the primary Care Coordinator for designated facilities within the region, typically those with lower skilled managed care volumeManage a direct caseload including comprehensive resident assessment, care planning, and discharge coordinationMaintain accurate and timely documentation for assigned residentsCoordinate directly with insurance providers for authorizations and continued stay reviews for assigned caseloadEnsure compliance with all regulatory requirements and facility policies for assigned residentsLeadership and Support : Provide guidance, mentorship, and professional development support to facility Care Coordinators within the assigned regionAssist in developing and implementing policies and procedures to ensure effective care coordination practicesSupport and monitor care coordination practices to ensure compliance with regulatory standards and best practicesServe as a resource for complex care coordination situations and provide solutions-oriented guidancePatient Care Coordination : Manage direct caseload at assigned facilities while supporting facility Care Coordinators in the assessment, planning, implementation, and evaluation of patient care plans at other regional locationsEnsure residents receive appropriate and timely care through both direct care coordination and oversight of facility care coordination processesCollaborate with interdisciplinary teams to facilitate comprehensive care and discharge planningBalance direct caseload responsibilities with regional support duties to maintain optimal resident care across all facilitiesQuality Improvement : Partner with facility Care Coordinators to identify areas for improvement in care coordination processesImplement strategies to enhance patient care and operational efficiency across the regionMonitor patient outcomes and satisfaction, using data to drive quality improvement initiativesEnsure compliance with all federal, state, and local regulations related to care coordinator and patient careMonitor and improve payer authorization capture to support patient needs and reduce hospital readmissionsCollaboration and Communication : Serve as a liaison between facility Care Coordinators, residents, families, healthcare providers, and external agenciesAttend monthly meetings with health plansSupport development of strong relationships with community resources to support residents' needsFacilitate effective communication between all stakeholders to ensure a seamless continuum of careIdentify and secure funding opportunities to support care coordination programs and initiatives.RequirementsEducation / LicensureCompletion of nursing training from accredited school or college OR Bachelor's degree in Healthcare Administration or related healthcare fieldValid CPR and BLS card (when applicable)Qualifications / ExperienceMinimum of 5 years case management / care coordination experience in skilled nursing facility, preferred preferredMinimum of 3 or more years as a licensed nurse, preferredExcellent verbal and written skillsKnowledge of Point Click Care and Net Health, preferredStrong mentoring and coaching abilitiesDemonstrated ability to influence and guide without direct authorityWorking ConditionsRegular travel required within assigned regionMay encounter frequent interruptionsMay be involved with residents, family and government agenciesMay be requested to work beyond scheduled working hours at timesIs subject to callback during emergency conditions (e.g., severe weather, evacuation, postdisaster, etc.)May be necessary to assist in the evacuation of residents during emergency situationsMay be exposed to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B virusesPhysical RequirementsMust be able to move intermittently throughout the dayWorking throughout the nursing areasRepetitive hand motionAbility to read fine print on progress notes and / or medical labels#J-18808-Ljbffr
