What are the responsibilities and job description for the Admissions Coordinator position at BirchPointe Health & Rehab?
Birch Pointe Health and Rehabilitation is currently looking for an Licensed Nurse (LPN or RN) for our Marketing & Admissions Coordinator.

The primary duties of this job include:
- Review resident referrals for appropriate placement
- Effectively communicate with discharge planners from hospital and other agencies
- Perform assessment of the resident at admission, upon change of condition and/or annually; create, review and update care plan and progress notes
- Assist resident’s family in coping with placement in a skilled nursing facility, physical illness, disabilities of the resident, and the grieving process
- Schedule, and interview patients/representatives to obtain required admission information, including identification records and copies, and signatures on all applicable admission paperwork
- Coordinate and work with the resident, family and other team members to schedule admissions, transfers, and discharges of residents as required
- Conduct job responsibilities in accordance with the Company’s Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standards and applicable state/federal laws
- Perform additional duties as assigned
Experience:
- Knowledge of practices applicable to a long-term care environment are preferred, but not required.
- Prefer, experience with Managed Insurance Authorization Process, but not required.
- Prefer Marketing experience, but a great attitude is all that is required.
- Prefer Point Click Care charting experience, but not required.
- Skill in directing and motivating facility staff in care of new admissions is a must.
- Must work cooperatively as a member of a team.
- Must have Positive attitude.
- Familiarity with Social Media (Facebook) helpful.
- Must communicate effectively with residents, their families and at all levels of the organization and maintain confidentiality.
- Must possess valid drivers license.

Work Hours:
- primarily Monday - Friday.
- Some after hours
- Some weekends
- Call phone required (supplied by company)
- Some travel, locally.
Required Education:
- Valid unencumbered LPN/RN License for state of Missouri or Multi-compact license.
