What are the responsibilities and job description for the Behavioral Health - Aftercare Coordinator position at BJC?
- Shift information: Day Shift - Monday – Friday (8 am - 4:30 pm)
- Team Dynamic: Working independently and working with Department Manager and other Aftercare Coordinators also collaborates with Supervisors from other facilities
- Location: Hybrid work schedule (2 days work from home – 3 days in the office). Can work out of the North Facility in Florissant on Graham Road or the 3309 South Kingshighway facility
- Ideal Skill Set: Must be able to use MS excel (operate pivot tables, maintain excel workbooks that have already been established)and after training, know how to use documents within Microsoft teams and our EMR (myAvatar)
- MUST HAVE skills for this position: Excellent time management and communication, ability to follow standard work
- Daily Job Responsibilities: Run various reports in EMR and translate them to various Excel workbook documents, upload to the Microsoft teams’ channel, document tasks in EMR

Overview
BJC Behavioral Health is a community health center that provides and coordinates behavioral health services for more than 8,000 seriously mentally ill adults and seriously emotionally disturbed children in St. Louis City, St. Louis County, St. François, Iron and Washington counties. As an Administrative Agent of the Missouri Department of Mental Health (DMH), BJC Behavioral Health serves as a major point of entry for people eligible for mental health services funded by DMH and is responsible for serving as gatekeeper to the public mental health system.

Role Purpose
Serves as a liaison between BJC Behavioral Health staff and inpatient staff during hospitalization for Healthcare Home clients across multiple hospital systems. This position will facilitate a smooth transition between hospitalization and returning home by coordinating care and alerting the care team to the follow up needs of the client. This position provides care coordination services designed to streamline plans of care, reduce hospital admissions, ease the transition to long term services and supports, and interrupt patterns of frequent hospital emergency department use.
