Our organization is seeking a Full-time Wraparound Facilitator with the corresponding experience requirements:
Salary: $55,000 - $57,000
GENERAL RESPONSIBILITIES
Responsible for developing and coordinating family-centered teams to meet the individualized needs of children and families in accordance with the Wraparound Milwaukee Model. Provide general case management services including: case planning, treatment planning, family conferencing, family focused/centered intervention, tenants of the model and documentation and ensures all services are maintained with an emphasis on culturally competent services.
Responsible for the development of an integrated treatment plan with the Child/Family Team. Must provide daily case management and leadership to ensure implementation of services and supports to the child and their family/caregiver, as agreed upon in the treatment plan. Provides facilitation, assessment, and case management services including all related documentation for services planned and provided. The position provides an advanced level of facilitation to support and enhance the efforts of children and their families/caregivers, the system workers, which are part of the support team, and other individuals in a variety of environments. This position works independently in the family’s home and a variety of community and/or treatment settings. This position works directly with families 24/7/365.
DUTIES AND RESPONSIBILITIES
· Completes initial three-day wraparound training within first 90 days of hire.
· Ensure Wraparound Milwaukee model integrity is adhered to in direct service and documentation standards.
· Utilize a strengths-based perspective to empower families.
· Conduct meetings with families and team members in accordance with model standards and productivity requirements. (1 ½- 2 hours)
· Provide direct intensive case management services as needed.
· Ensures timely completion and submission of appropriate documentation (EBO,REDCap, etc).
· Complete Family Status Report (FSR) at intake, quarterly and termination of services
· Adhere to agency reporting requirements.
· Maintain an active caseload of at least 8-10 families, but may not exceed 12 if families are in transition
· Complete a minimum of two MDHHS Wraparound trainings per calendar year.
· Serve as a liaison or advocate for families with other community providers.
· Perform other duties as assigned.

QUALIFICATIONS
Minimum Bachelor’s Degree in Social Work or related field; must have experience in family-focused and family-centered practice and strength-based services; must have experience in completing comprehensive assessments, treatment planning and report writing. Complete required trainings to be classified as a Child Mental Health Provider (CMHP).
All candidates must demonstrate strong computer literacy; strong documentation and oral communication; ability to work effectively as a member of a multidisciplinary team; ability to schedule appointments at dates and times flexible to the student/family, including non-traditional work hours. Qualified candidates will possess experience working with diverse low-income communities, and sensitivity to the needs of culturally diverse urban adolescents and their families; experience working with at-risk youth; as well as familiarity with the basic concepts of positive youth development. Required pre-screening qualifications include: satisfactory criminal records check and other satisfactory background clearances.
