POSITION SUMMARY: Manages and coordinates Care Coordination services within the health system, providing person-centered care to patients in various settings. Collaborates with staff to ensure high-quality services and supports individual health care needs. Works with clinical leaders on protocol development and may operate in clinics or training environments.
- EDUCATION: Registered Nurse, BSN preferred.
- CERTIFICATION/LICENSES: Current New Mexico Nursing License.
- SKILLS: Knowledge of health care policies, ability to coordinate services, strong communication skills, and cultural sensitivity.
- EXPERIENCE: Minimum two years in health care, with a focus on quality and care coordination preferred.
- SUPERVISION: Reports to Manager of Population Health and Director of Ambulatory Patient Care Services.
- ENVIRONMENT: Various health care settings with potential exposure to bloodborne pathogens.
- PHYSICAL REQUIREMENTS: Mobility in care settings, good vision, dexterity, and excellent customer service skills required.
