We are seeking a Physical Therapist to provide home health, palliative, and hospice care in the Chicagoland area. Enjoy great flexibility with a per diem pay rate of $75-$140. Responsibilities include assessing patients' therapeutic needs, developing care plans, and instructing patients and caregivers on therapeutic appliances.

Qualifications

  • Baccalaureate or master's degree in physical therapy
  • Current Illinois Physical Therapist license
  • Current CPR certification
  • Valid IL driver's license and auto insurance
  • Home Care Home Base experience preferred
