What are the responsibilities and job description for the Care Navigator Department Supervisor (RN) position at Blackstone Valley Comm?
The nurse supervisor is responsible for the daily management and coordination of the Care Team Navigator Department and the NCMs. Responsibilities include the maintenance, monitoring, and reporting of activities and data that impact the quality and cost of care provided by BVCHC.
EDUCATION, EXPERIENCE, & SKILLS
Registered Nurse Required
Knowledge of PCMH, MU, and UDS preferred
Familiarity with healthcare, especially FQHCs, as well as the underlying concepts underlying an accountable care organization
Must be proficient in Excel, have working knowledge of statistics, and demonstrate ability to learn new systems
OTHER REQUIREMENTS
Reliable transportation
Cultural sensitivity necessary to work with a diverse patient and staff population
Aptitude for effective, timely management of projects and employees within scope of department
Ability to work independently and collaboratively
Knowledge of computers and medical records required
Knowledge of Microsoft Suite preferred
Strong communication skills, both verbal and written
