Job Responsibilities
- Provide professional social services to Department of Social Services clients in assigned programs, including child protective services, adult services, and foster care services.
- Manage assigned caseloads, maintain accurate records, and accurately record services provided.
- Assess client needs, prepare, implement, and monitor care plans, making changes as necessary.
- Refer clients to other department divisions and outside agencies for services, assist clients in accessing needed services, and provide transportation as needed.
- Conduct home visits to perform initial assessments, monitor progress, and ensure clients are receiving necessary care and assistance.
- Attend staff meetings to discuss and coordinate cases, communicate with other professionals to coordinate client care, and conduct case audits.
