- Blessed Hands PNCC is looking for passionate coordinators in the Milwaukee County area only.
- Coordinators will provide social services and assistance to improve the social and psychological functioning of families and to maximize the family well-being and functioning.
- Coordinators will need to do outreach to find members to provide services.
- This is a home visiting program and requires you to visit families in their environment at least once monthly and the rest will be via phone.
- Pay is based on how many members contractor has obtained during their own outreach
- This is a 1099 position with Blessed Hands Prenatal Services, LLC.
- Must live and reside in or near Milwaukee, WI County.
Duties
- Interview clients individually, in families assessing their situations, capabilities, and problems to determine what services are required to meet their needs.
- Conduct outreach for pregnant women in need of services that aligns with the State of Wisconsin Prenatal Care Coordination Services.
- Support families in identifying resources regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical care and provide referrals as needed
Requirements
- High school diploma or GED required and 0-2 years of relevant professional experience required, or equivalent combination of education and experience.
- Prior case management experience, preferred.
- Prior customer service experience, preferred.
- Experience working with low-income and diverse populations, preferred.
- Microsoft Proficiency, preferred
- Attend monthly meetings
- Conduct outreach to identify women that qualifies for services
