Position Summary:

Bloom for Women, Inc is a non-profit organization dedicated to empowering survivors of sex trafficking and sexual exploitation in the Lehigh Valley. Reporting to the Residence Manager, Bloom’s Case Manager will work closely with the participants of the organization to identify strengths and challenges, formulate goals to achieve healing, and work towards independence and leaving a life of violence. The Case Manager is responsible for ensuring that the medical, treatment, social and emotional needs of the participants are met. The Case Manager will be responsible for providing and coordinating ancillary services directly related to individual participant goals.

The Case Manager position is considered 'safety-sensitive' and is subject to random drug screening. As a safety-sensitive role, participants rely on Case Managers for transportation and daily tasks, which require maintaining a clean drug screen

Responsibilities:

· Work directly with participants who have experienced sex trafficking, sexual exploitation, and sexual abuse.

· Engage with individuals who may also have a history of substance use, violence, criminal justice involvement, and homelessness.

Core Responsibilities:

  • Develop and implement individualized care plans that support healing and independence, through trauma-informed engagement and goal-setting with each participant.
  • Coordinate and advocate for services including medical, mental health, legal, and social supports, ensuring participant needs are met through internal resources and external partnerships.
  • Maintain safety and compliance, including transporting participants as needed and adhering to Bloom’s safety-sensitive standards, such as passing random drug screenings and ensuring participant well-being during all interactions.
  • Complete timely and accurate documentation including intake assessments, progress notes, incident reports, and ongoing updates to care plans, in accordance with organizational policies and confidentiality standards.

Additional Duties:

· Participate in the intake process for new participants, including orientation, search of belongings, drug testing, and initial rapport-building.

· Intentionally foster safe and positive relationships with participants by being consistently present in community circles, family dinners, one-on-one sessions, and other activities.

· Assist participants in connecting with ongoing resources such as medical, mental health, recovery, and other supportive services.

· Schedule and facilitate weekly check-ins with participants to set recovery goals, review planners and passes, and address any concerns.

· Maintain a trauma-informed and strength-based approach in all interactions.

· Follow all daily program schedules as directed by the Residence Manager.

· Ensure program rules, expectations, and residence guidelines are consistently upheld.

· Maintain detailed case notes and documentation in the case management software.

· Conduct data entry and perform ongoing assessments to track progress.

· Monitor participants’ medical and mental health needs and coordinate appropriate services.

· Support and occasionally lead life skills programming, including budgeting, job readiness, meal planning, resume writing, and other key areas.

· Facilitate emergency drills and respond to safety needs appropriately.

· Administer random drug and pregnancy testing.

· Participate in a rotating weekend on-call schedule.

· Contribute to outreach and direct-to-victim efforts when needed.

· Report maintenance needs to the Residence Manager and assist with small upkeep tasks such as changing filters, touch-up painting, and organizing donations.

Qualifications:

· Bachelor’s Degree in Social Work or related field. In lieu of a degree, relevant life and work experience will be accepted

· Knowledge of sexual exploitation, including sex-trafficking, victimization, and women’s issues

· Knowledge of substance use disorder and long-term negative effects, including physical health problems as well as mental illness

· Ability to work flexible schedule going between Bloom’s residences

· Strong ability to maintain confidentiality and report critical information in a timely manner

· Strong written and oral communication skills

· Excellent interpersonal skills

Job Type: Full-time

Pay: From $20.00 per hour
