- Understands and creates patient care plans
- Updates patient specialty and comprehensive care plans to meet the needs of the patients and providers
- Coordinates care with community partners and teammates
- Coordinates and tracks transitions of care
- Provides exceptional professional customer service to patients, families and community partners
- Collaborates and coordinates with the Interdisciplinary Care Team (IDC) to provide excellent patient care
- Supports and implements quality improvement programs
- Strong computer skills and performance tracking
- Ability to multi-task at a very high level and manage time efficiently
- Strong problem-solving abilities
- Excellent phone/customer service skills
- Strong Clinical Background
- Flexibility and adaptability to changes
- Ability and willingness to expand clinical and general healthcare knowledge
- Strong critical thinking skills
- Self-motivated
- Experience in healthcare coordination, case management or similar role
- Experience in out-patient/ambulatory/home health environment
- Active professional license (RN, LPN)
- Minimum 3 years of clinical experience
