JOB DESCRIPTION
The Foster Care Family Consultant will ensure that children living in Bloom foster homes are safe and receiving the individualized case management services they need for healing, well-being and permanency. The Foster Care Family Consultant will maintain exceptional service relationships with Bloom foster families while maintaining a caseload of 12-15 children.
PRIMARY RESPONSIBILITIES:
• Assist in the development and management of individual treatment plans with the input of the family, legal guardian or significant others.
- Provide or coordinate therapeutic supportive services and/or resources to Bloom foster children with mild/moderate/severe emotional, behavioral, or physical needs.
- Advocate on behalf of the child and family with social service agencies, court, police, medical, school and mental health professionals.
- Establish a formal process for reviewing cases and communicating information between agencies and referral sources.
- Conduct home visits, monthly Quality Assurance checks and provide ongoing assessments to ensure foster homes remain compliant with foster care regulations.
- Comply with documentation requirements and mandated deadlines established by DHS, OPM, ORCC, and Bloom in GA SCORE, GA SHINES and Childtrax.
- Provide crisis stabilization services and participate in the on-call, after-hours cell phone rotation in order to respond to foster care referrals and provide emergency assistance to Bloom foster parents and foster children.
- Assist with placement of children into Bloom foster homes by coordinating arrangements between the state placement worker and the Bloom foster parent.
- Participate in individual supervision, case conferences, staff meetings, case planning meetings, and team meetings.
- Provide training and resource development to foster parents specific to the needs of their respective foster child(ren).
- Adhere to all mandated and critical incident reporting guidelines.
- Assist with other miscellaneous duties as assigned.
This is a full- time position requiring flexibility of schedule. Some weekends and nights required. Travel throughout the metro Atlanta area, Fayette County, and surrounding areas.
JOB REQUIREMENTS
- Master’s or Bachelor’s Degree in social work, psychology, education, counseling, or a related field.
- At least two years’ experience in foster care case management preferred.
- Must be at least 21 years of age and possess a valid Georgia Driver’s License.
- Able to pass a criminal records check, child protective services check, and motor vehicle check.
- Experience working in GA Shines and GA Score preferred.
- Strong oral and written communication skills.
- Must have a caring and positive attitude and be passionate about helping children and families in the foster care system.
- Computer efficiency with strong analytical skills and in- depth knowledge of child welfare best practices.
- Must be culturally competent, a team player and have high standards of excellence.
